• Providing outpatient preventive and chronic disease management
• Provide care across clinic and post-acute settings
• General primary care evaluation and management
• Preventive care services
• EMR: Cerner / Oracle

Requirements

• Family Medicine Certification/Training Appropriate for NP or PA Practice - [REQUIRED]
• Active Vermont License - [PREFERRED]
• Providers with Lapsed Board Certification - NOT ACCEPTED
• Primary Care Experience - PREFERRED
• Adult Patient Care Experience - REQUIRED
• Geriatric Patient Care Experience - REQUIRED
