- Board certified (PA-C; must have passed PANCE via NCCPA) and NP (ANCC or AANP or equivalent approved by IL Board)
- Certifications required: ACLS, ATLS, PALS
- active IL license and DEA/CSR
- Case logs required: past 24 months
