- CRNA practices independently
- General, Ortho, GI, Urology, ENT, OB. No Head/Heart. General, GI, and Ortho make up majority of cases.
- coverage needed 6/16-6/18 and 7/13 - 7/17 and every monday in July
- Active IA CRNA License or Active Compact License
- Clean Malpractice/Background History - [REQUIRED]
