• Provider Credentials: MD or DO
• Board Status: Board Certified or Board Eligible in Family Medicine
• Certifications Needed: Current and valid medical license in the state of Indiana

• Job Class: Permanent
• Estimated Start Date: As soon as credentialed
• Estimated Privileging Timeline: Based on credentialing process
• Schedule Details: Full-time schedule; specifics to be discussed during interview
